Clarified that I cannot get the OnSaveHandler to fire in ANY Document Library (existing and customized or brand new and totally not customized) -BUT if I use the custom fields in a regular SharePoint list then the OnSaveHandler DOES fire!

I discovered that this issue only seems to occur when my custom fields are used in a Document Library but work fine in regular SharePoint lists.
