Have you ensured that there is no timer job or some other custom code, from where your workflow is triggered explicitly ? I am not sure how you will do that. But it is also a possibility for triggering a workflow.

Why is it happening for only few documents then ? :) It is any of the services, then error should be for all xlsx files.

One more update, in my environment I can't see the title column even for "Item" content type.

After executing few power-shell scripts i found that the "Title" column is not part of the "MyContentType.Fields" collection, but it is part of "MyContentType.FieldLinks". So, it is not getting displayed in the list of columns for "MyContentType". On my environment i can see a custom content type where "Title" column is listed in the list of columns for that content type.

I downloaded the .wsp file and extracted its content. It has Elements.xml, Feature.xml, manifest.xml, Workflowname.xoml, Workflowname.xoml.rules........ So, it seems to be a SPD workflow.
