@johnG No, create them at site level. And in the list in the subsite, change CT to the one you created at site level

@johnG For every custom CT, inherit from the issue CT at sirte collection level. In the custom list, change the default CT to the new one, and you can still manage them "globally"

@johnG the recommended approach is to have content types at site level and make a CT inheritance structure. Then allow for PMs to edit these site content type.

@user2068804 From SQL Server you could use the profiler tool as well, but it needs heavy filtering. And it's not the same as Get-SPDatabase which will output all the databases in one time. sp_who outputs connections, and you usually have more than one connection to each database.

@Mark Unfortunately not out of the box. You could hide it with inline CSS or inline JavaScript, but for this you need to make sure that those who should be able to change can do so. You can solve it through SPServices on CodePlex
