this updates an app, but where does it say where to add columns to a content type in an app, how does this really work, does it work the same as the feature upgrade toolkit from Chris O Brien?, can I set a begin version and end version and decide what to do on each upgrade ?

with this info, then I guess best practice would be to deploy content types and lists to the host web, and then leave the "business logic" in apps using rest calls to the host web as COB explains on his articles.

with 30 millions items, 1 server would not be enough and then you need to scale out. Please check the farm topologies for recommendation based on limits, blogs.technet.com/b/schadinio/archive/2010/08/18/…, look ath the small and medium topologies. Your Sharepoint will be way too slow specially if the crawl components are on the same WFE.
