The relationship dance from column to crawled property to managed property is not an easy one until you have worked with it a while. Search is a different beast more or less fully decoupled from the object model of SP.

ikae would not work, as it's term by term contains, not letters contained in a word. What you want is pre/postfix wild cards, which is not supported by the SP index afaik.

I haven't have time to look into this, but could you try to create a console application which uses your code, and run the same logic. Call readlist, and readitem. And then check the memory usage of your console app.

Can you say something about how many unique permissions you have for a failing item? And maybe you need to set both params...which is weird.
