How to create a survey in Sharepoint which supports sub options in choice item?

Question

Now need to create a survey according to customer's requirement like following:

Question: Please choose the level of interaction between you and customer every day?

• A. Call received none, 1-2, 3-5, 5-10, 10+ (these are options too)
• B. Call placed none, 1-2, 3-5, 5-10, 10+
• C. Email received none, 1-2, 3-5, 5-10, 10+
• D. Email sent none, 1-2, 3-5, 5-10, 10+

I know we can split the choice into separate items to complete it, but customer doesn't want to do that. Does anyone have idea on this?
