I recently started working in a large governmental corporation. There are many offices throughout the organization and most of them have a SharePoint site that serves one purpose or another. Mostly, these sites are used to track online forms that are designed in InfoPath.

Usually, when a new form is submitted, there is a workflow that serves some purpose or another (like sending a notification email to the appropriate person). Now the problem is, once the site is made live and gets used by the client, the workflows don't start consistently. Say for every 10 forms uploaded to the same library, 1 or 2 wont start. It doesn't end with an error. It simply doesn't start. If I select the item and start it manually, there are no problems. These forms can be filled in by anyone inside the organization with roughly 22k employees. I have heard my colleagues complain similarly about the workflows.

I just want to know; is there any other organizations out there experiencing similar problems?
